Our Service Planning Coordinators will work with you to create a plan that brings together all of your service providers. Our goal is to make the process easier for families, ensuring you receive a seamless and family-focused experience without dealing with confusing or overlapping services.

Here’s what you can expect from our Coordinated Service Planning:

• A dedicated Service Planning Coordinator who will be your main point of contact, responsible for developing and monitoring your child’s plan.

• You won’t need to repeat your story or goals to multiple providers. We’ll make sure everyone’s on the same page.

• You’ll have one plan that’s designed specifically to meet your child and family's strengths, needs, and goals.

• The process will focus on your family’s unique situation, recognizing that you know your child best.

• All service providers working together toward common goals outlined in the plan.
